These Terms and Conditions are revised as of April 1, 2020.
Snoasis Medical, LLC (“SNOASIS”) accepts product orders through a variety of mediums. To place an order by phone, please contact us toll free at (866) 521-8247 or at (303) 242-8075. Email orders can be sent to email@example.com. Fax orders can be sent to (720) 259-1405. Orders can also be placed online through our website. To order online, you will need to register with the website. Registrations can take up to 48 hours to approve.
Snoasis utilizes a tier-based approach for pricing BioXclude®. All purchases move customers toward a better price tier. Our price tier calendar year runs from February 1 – January 31. Tier assignment is based on cumulative BioXclude purchases from the previous year. Once the threshold for a specific price tier is reached, that price tier is locked through the end of the year. New Customers: Orders of 10 or more grafts receive Black pricing. Orders of fewer than 10 grafts receive Blue pricing. Both options can be any combination of sizes. Orders of 10 may receive a full refund after evaluation of up to two (2) grafts.
For all other orders, please contact us about bulk specials and opportunities to lock in tiers.
Product may be returned without penalty if there is evidence of contamination, physical non-conformance, damage to packaging, incorrect labeling, if the order was received in error or does not match order. Return requests for any other reason are evaluated on a case-by-case basis, so long as the time from purchase has not exceeded one year. Returns may be assessed a 20% restocking fee. All placental tissue is sourced in the United States with informed consent from pre-screened mothers who choose to donate their placenta following elective caesarian section deliveries. The tissue is procured, processed, and distributed according to standards and regulations established by the American Association of Tissue Banks (AATB) and the United States Food & Drug Administration (FDA). All tissue must meet stringent specifications during donor screening and laboratory testing to reduce the risk of transmitting infectious disease.
